GENERAL INFORMATION ABOUT OUR OFFICE

    We have an automated telephone system. There are prompts to help you reach
    our 1)Emergency Line, 2) Sick or Urgent calls line, 3)Future appointments line,
    4) Prescription refill line, 5) Referral line and 7) Billing line.

         Answering Service Number: When you call the office main number you will
^ be given the answering service number ifthe office is closed. Call the answering

          service only when we are not in the office (See office hours below). Please try

          the main office telephone number first.

^.  We are committed to provide you, our patients, with the best and most timely
    care that we can. For that reason in addition to our regular business hours the
    office is open Monday - Thursday evenings and Saturday and Sunday mornings.
    Calling early during these time periods will allow us to provide you with a time

    that is convenient.

            Office Hours:
           Monday - Thursday: 8:00 a.m. to 8:00 p.m.
           Friday: 8:00a.m. to 5:00 p.m.

           Saturdays & Sundays: 8:30 a.m. to 12:00 noon (Urgent care)

          Evenings:

^ Monday through Thursday the on-call physician and one of our nurse

           practitioners will be in the office from 6:00pm to 8:00pm. Sick visits and a limited
           number of well child visits will be scheduled during that time. If an evening
          appointment is missed another evening appointment for that family will not be

^ scheduled due to the fact that the evening appointments are limited and in

           demand. Parents scheduling visits during these evening hours should keep in
          mind that the on-call physician could be called out for an emergency at any time
          and it might be necessary for them to wait or reschedule their appointment.

* Nights:

           There is a pediatrician available for urgent matters after the office has closed for
           the day. This booklet has been written to help you to begin treatment for minor

^ iilnesses at home. Please refer to it before placing a call or taking your child to

           the emergency room. If you still have a question or concern, please call the
          answering service number. For parents with an urgent question or concern after
           10:00pm we provide a night time service of specially trained nurses who will help
           you assess your child and decide if they need to be seen before the office opens

* in the morning. If your child is seriously ill and needs to be seen by a physician

           you will be asked to bring your child to the emergency room.

^ Weekends:

           There is a pediatrician available for urgent matters over the weekend. We
           understand that it is often possible to wait through the night to talk with a
           physician but that it may not be possible to wait through the weekend. For this
           reason, pediatricians from our office are on call during the weekend and they will
          see patients with urgent problems in the office on Saturday and Sunday
