VII. WELL CHILD CARE AND IMMUNIZATIONS:                                             ^

A. Well Child Care Schedule

There is a great emphasis today, especially in pediatrics, to practice "preventive

medicine", i.e., to anticipate and prevent disease, or at least provide early

detection. Following is the recommended well child screening schedule.

This schedule is of course, flexible and is designed as a guideline to be followed
whenever possible. Children with specific problems or difficulties will need to

have their routine care individualized.

Annual visits for ages 6 to 20 years are encouraged.

At each well child visit we screen every child for delays or differences in
functioning in physical, cognitive, language, psychosocial and behavioral
development. Screening questionnaires are given to the parents at each visit.

Immunization Notes:

An immunization schedule is available as a separate handout in our office. New
vaccines are being created and recommendations change fairly frequently so we
have decided to remove the schedule from this booklet in an effort to keep the
schedule as up to date as possible.

We are able to determine what immunizations your child will need for travel to      ^
foreign countries. Please call our office as soon as you know that you will be

traveling outside of the United States. You will need 4-6 weeks for most

immunizations required for travel to become effective. There are also travel

immunization clinics at Memorial Hospital (for ages 15 yr and older) (508) 334-
